-A photo ID (if you have one)

-Social Security numbers for everyone in your household OR documentation numbers for legal immigrants

-Employer and income information for every member of the household (pay stubs, W-2 forms)

-Policy numbers for any currently-held insurance policies

-Physician and prescription information
